What to Include
Please include the names of all parties involved, including the provider, patient, and insurance policyholder. Specify which party you believe is committing the fraud. Identify the dates of service or items in question. Describe, in as much detail as possible, why you believe a fraud may have been committed.

If possible, please include your name, telephone number, and address in case we need to contact you during the investigation.

Montana law provides immunity from liability to any person who cooperates with, furnishes evidence to, or provides information regarding suspected insurance fraud. You may remain anonymous.
